Identifying and Describing the Impact of Cyclone, Storm and Flood Related Disasters on Treatment Management, Care and Exacerbations of Non-communicable Diseases and the Implications for Public Health
Conclusion Cyclone, flood and storm related disasters impact on treatment management and overall care for people with NCDs. This results in an increased risk of exacerbation of illness or even death. The interruption may be caused by a range of factors, such as damaged transport routes, reduced health services, loss of power and evacuations. The health impact varies according to the NCD. For people with chronic respiratory diseases, a disaster increases the risk of acute exacerbation. Meanwhile, for people with cancer, cardiovascular diseases and diabetes there is an increased risk of their illness exacerbating, which can ...

Pharmacological interventions for the prevention of bleeding in people undergoing definitive fixation or joint replacement for  hip, pelvic and long bone fractures
CONCLUSIONS: We cannot draw conclusions from the current evidence due to lack of data. Most published studies included in our analyses assessed the use of tranexamic acid (compared to placebo, or using different routes of administration). We identified 27 prospectively registered ongoing RCTs (total target recruitment of 4177 participants by end of 2023). The ongoing trials create six new comparisons: tranexamic acid (tablet + injection) versus placebo; intravenous tranexamic acid versus oral tranexamic acid; topical tranexamic acid versus oral tranexamic acid; different intravenous tranexamic acid dosing regimes; topical ...

Maternal and perinatal outcomes following a diagnosis of Hodgkin lymphoma during or prior to pregnancy: a systematic review
CONCLUSIONS: Rates of most adverse pregnancy outcomes among women with a previous/current HL diagnosis are not increased significantly compared to the general pregnant population. Women with HL during pregnancy may have a higher PTB rate and increased likelihood of VTE, anaemia and blood transfusion, however small study numbers and low-very low GRADE certainty of findings preclude conclusions.PMID:36424902 | DOI:10.1111/1471-0528.17347

Question 2: Does the timing of central line placement in relationship to the initiation of acute lymphoblastic leukaemia therapy change the risk of thrombosis or infection?
Question Does the timing of central line placement in relationship to the initiation of acute lymphoblastic leukaemia (ALL) therapy change the risk of thrombosis or infection? Clinical scenario You are the haematology registrar doing ward rounds with your consultant. A 3-year-old with newly diagnosed ALL is expected to start induction chemotherapy in a few days. Your consultant questions if we should consult surgery for line placement prior to induction chemotherapy or if we should wait until remission is achieved. Structured question In children with ALL (population), does placing a central line at the end of induction (i...

Efficacy and Safety of Direct Oral Anticoagulants in Patients with Cirrhosis-- Single Institution Experience
Conclusions:Our study is the largest series of patients with cirrhosis treated with DOACs. Bleeding complications with DOAC use in this population is high. Providers considering starting DOACs in patients with cirrhosis should consider the risk of bleeding in this population.DisclosuresNo relevant conflicts of interest to declare.

Cancers, Vol. 14, Pages 1937: In Search of the Appropriate Anticoagulant-Associated Bleeding Risk Assessment Model for Cancer-Associated Thrombosis Patients
sabelle Mahé Patients with venous thromboembolism events (VTE) in the context of cancer should receive anticoagulants as long as the cancer is active. Therefore, a tailor-made anticoagulation strategy should rely on an individualized risk assessment model (RAM) of recurrent VTE and anticoagulant-associated bleeding. The aim of this review is to investigate the applicability of the currently available RAMs for anticoagulant-associated bleeding after VTE in the CAT population and to provide new insights on how we can succeed in developing a new anticoagulant-associated bleeding RAM for the current medical care of CAT pa...

Fibrinogen: A Circulating Factor in Search for Its Genetic Architecture.
Abstract Fibrinogen (Coagulation factor I) is a major player in thrombus formation; it is cleaved by thrombin to form fibrin, which is the most abundant component of a blood clot(1). Beyond the role played in the coagulation and cardiovascular diseases (CVD), fibrinogen is a proinflammatory factor in autoimmune and inflammatory diseases (such as rheumatoid arthritis, vasculitides, inflammatory bowel disease, multiple sclerosis, chronic obstructive pulmonary diseases and kidney disorders and post-transplant-fibrosis)(2) as well as in several types of cancer(3). Fibrinogen has been demonstrated to interfere with the...

Engineering novel anticoagulant proteins by motif grafting.
Abstract Structure-based rational design has been considered as a promising approach to design novel proteins. For this purpose, we designed artificial anticoagulant proteins that are able to target Factor Xa (FXa) using a functional motifgrafting approach. The motif corresponded to the residues Cys15 to Cys42 of Ancylostoma caninum anticoagulant peptide 5 (AcAP5), a potent FXa inhibitor. By screening of the Protein Data Bank (PDB) using Vector Alignment Search Tool (VAST, search for three-dimensional scaffolds in protein structures), we screened scaffolds as hosts to reproduce the functional topology of this moti...

Feasibility and cost effectiveness of stroke prevention through community screening for atrial fibrillation using iPhone ECG in pharmacies. The SEARCH-AF study.
Abstract Atrial fibrillation (AF) causes a third of all strokes, but often goes undetected before stroke. Identification of unknown AF in the community and subsequent anti-thrombotic treatment could reduce stroke burden. We investigated community screening for unknown AF using an iPhone electrocardiogram (iECG) in pharmacies, and determined the cost-effectiveness of this strategy.Pharmacists performedpulse palpation and iECG recordings, with cardiologist iECG over-reading. General practitioner review/12-lead ECG was facilitated for suspected new AF. An automated AF algorithm was retrospectively applied to collecte...

Finite Element Analysis in Asymptomatic, Symptomatic, and Ruptured Abdominal Aortic Aneurysms: In Search of New Rupture Risk Predictors.
CONCLUSIONS: From different FEA derived parameters, PWRI distinguishes most precisely between asymptomatic and symptomatic AAA. If elevated, this value may represent a negative prognostic factor for asymptomatic AAAs. PMID: 25542592 [PubMed - as supplied by publisher]

Prosthetic heart valves in pregnancy, outcomes for women and their babies: a systematic review and meta‐analysis
ConclusionsWomen with heart valve prostheses experienced higher rates of adverse outcomes than expected in a general obstetric population; however, lower than previously reported. Women with bioprostheses had significantly fewer thromboembolic events compared to women with mechanical valves. Women should be counselled pre‐pregnancy about risk of maternal death and pregnancy loss. Vigilant surveillance by a multidisciplinary team throughout the perinatal period remains warranted for these women and their infants. Tweetable abstractMetaanalysis suggests improvement in #pregnancy outcomes among women with #heartvalveprostheses.
